In 2015, Medicare began reimbursing physicians for chronic care management for patients who had two or more chromic conditions under CPT code 99490. Which means Medicare will reimburse the physician for spending twenty minutes of non face-to-face time with the patient. But a complicated process discouraged physicians from providing this service, leading to only 1.5% of forty-million eligible patients enrolled into the program.
